The Root Cause Analysis (RCA) Toolkit and eLearning Programme (RCA Toolkit) has been created by the NPSA as a tool for guiding NHS staff through the process of conducting an RCA investigation. The RCA Toolkit is supplied free of charge to NHS staff, and its use by you is subject to these Conditions of Use.
I acknowledge that the copyright and all other intellectual property rights contained in the RCA Toolkit belong to NPSA, and confirm that I understand that I am permitted only to download, store, copy or use (Use) all or any part of the RCA Toolkit for my own personal use and research, and/or that of a national health service body, and that this Use is on condition that I acknowledge NPSA as the source.
I understand that unless I receive the NPSA's prior written consent, I understand that I must not Use any of the RCA Toolkit for the benefit of any other organisation and/or purpose (whether commercial or otherwise), nor may I publish, re-transmit, redistribute or make available to any third party the RCA Toolkit or its content in any media.
I understand that the RCA Toolkit is for information purposes only, and that the RCA can be distressing to those involved (whether in the conduct of the RCA, giving statements or other assistance) and therefore any use I make of it is at my own risk. I agree that the NPSA shall have no liability or responsibility for any loss or damage (other than personal injury or death caused by the NPSA's negligence) suffered by my using the RCA Toolkit. I also undertake to draw these Conditions of Use to the attention of any other persons involved in an RCA, who may be unaware of them.
