It is important that you acknowledge and understand the following to report to the NRLS:


  1. NRLS Reporting has been created to enable you to upload incident reports to the NRLS to help ensure continued learning about patient safety.

  2. The NRLS does not keep information identifying individual patients or healthcare staff. You should ensure that no personal identifiable information is uploaded to the NRLS (any such information will be removed).

  3. Regarding incident reports submitted via the online reporting form (eForm), the data will be kept in its submitted form for 30 days and may therefore contain personal identifiable information. If this is the case, please handle this data in accordance with NHS and local policies on information security.
